This role is primarily based in a remote setting, allowing for flexibility while ensuring that you remain connected with your colleagues and our clients. • Act as a liaison between clients, families, and outpatient care providers to ensure coordinated care and support.
• Engage effectively with insurance companies for pre-certification and utilization management, facilitating seamless access to necessary services.
• Participate in discharge and aftercare planning, ensuring clients have a comprehensive roadmap for continued support and stability.
• Provide therapeutic meal support, fostering a healthy relationship with food and addressing emotional eating patterns in a safe and non-judgmental manner.
• Deliver empathetic, affirming, and culturally competent care tailored to the unique circumstances of each client, acknowledging their intersecting identities.

In this role, you will be primarily working with older adults, who often face complex mental health needs alongside physical health challenges. You will enable them to navigate emotional and psychological difficulties related to aging, loss, and transitions in their care. Your approach will honor their life experiences while providing tailored interventions that empower them to enhance their quality of life.

• A Master’s degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in the state of Illinois is essential.
• A strong background in clinical assessment, diagnosis, and therapeutic intervention is necessary.
• Excellent communication and interpersonal skills to connect effectively with clients, families, and interdisciplinary team members.
• Demonstrated capability to work independently while also contributing effectively as part of a collaborative team.

• Prior experience working with older adults, particularly in hospice or palliative care settings, is highly desirable.
• Experience with electronic health record (EHR) systems and documentation processes in a clinical environment.
• Familiarity with therapeutic modalities tailored to the unique needs of the aging population.
