Key Responsibilities
  • Conduct psychosocial assessments for individuals, adolescents, families, and groups, tailoring interventions to meet the unique needs of each client.
  • Perform comprehensive initial evaluations, gathering detailed information for effective triage and care planning.
  • Collaborate closely with interdisciplinary teams through regular meetings and documentation, ensuring seamless communication regarding treatment strategies and client progress.
  • Utilize knowledge and skills to cater to the physical, psychological, social, educational, and safety needs of clients across all age groups.
  • Manage and coordinate case management duties for assigned patients, engaging with hospital and community resources to provide comprehensive care.
  • Document all interventions and client progress clearly and promptly, adhering to departmental policies to ensure ethical practice.
  • Implement Cognitive Behavioral Therapy (CBT) and other evidence-based therapeutic modalities to guide patients and families in developing their personalized service plans.
