• Conduct thorough patient intakes, focusing on diagnostic assessments and functional evaluations, to develop tailored treatment plans that align with established protocols of the Federally Qualified Health Center (FQHC).
• Collaborate with primary care providers to identify, manage, and treat complex mental health and psychosocial issues.
• Deliver brief, targeted interventions for clients needing immediate support and facilitate their access to necessary mental health services.
• Monitor patient progress through regular follow-ups, evaluating and modifying treatment strategies as necessary.
• Create and implement relapse prevention plans that empower patients to maintain stability and improve their overall quality of life.
• Help identify at-risk patients and develop actionable strategies to mitigate the escalation of their conditions.
• Document all patient interactions comprehensively, ensuring clear and accurate reporting within clinical records.
• Provide consultations and educational resources to patients to enhance self-management and promote healthier lifestyle choices.
• Participate in team meetings to discuss behavioral health service delivery and contribute to continuous program evaluation for effective outcomes.

• A minimum Master's Degree in Clinical Counseling from an accredited program.

• Strong insight into developmental psychopathology, DSM-IV diagnoses, and evidence-based therapeutic methods.
• Demonstrated ability to utilize community resources effectively to support patient needs.
• Excellent interpersonal skills for establishing relationships with patients, families, and stakeholders.
• Strong organizational skills with the ability to maintain accurate clinical records and prepare reports.
• Ability to work collaboratively within an interdisciplinary team and communicate effectively.

• Bi-lingual proficiency in English and Spanish is preferred.
