In this role, you will conduct comprehensive assessments, develop treatment plans, and implement therapeutic interventions tailored to each patient’s unique biopsychosocial needs.

About the Organization

Key Responsibilities

  • Facilitate access to prescription assistance programs through collaboration with community resources and pharmaceutical organizations, ensuring patients receive medications required for their treatment.

  • Act as a patient advocate, effectively communicating with community agencies and healthcare providers to coordinate services and support.

  • Maintain accurate and timely documentation of patient records, preparing necessary reports and written communications that comply with organizational policies and regulatory standards.

  • Coordinate transportation assistance for patients who may require support in accessing appointments and services outside of telehealth sessions.

  • Participate in regular clinical and team meetings, contributing insights during case conferences to enhance collaborative care planning.

Patient Population / Client Focus

Your work will primarily focus on a diverse client base, including individuals dealing with anxiety, depression, trauma, and other mental health disorders. Many patients are referred through community health agencies, providing you the opportunity to address social determinants of health that impact their well-being. Empathy, cultural competence, and a strength-based approach will be key in fostering therapeutic alliances.

Qualifications
  • A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling from an accredited institution is required.

  • Possession of one of the following active or independent licenses: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

  • A minimum of two (2) years of post-graduate clinical experience is essential, demonstrating a solid foundation in providing therapeutic services.

Preferred Experience
  • Experience working in telehealth or virtual therapy environments is highly valued.

  • Familiarity with case management practices and community resource navigation will enhance patient care delivery.

We offer a competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with experience and qualifications. In addition, our benefits package includes:

  • Comprehensive health, dental, and vision insurance to ensure your well-being.
  • Flexible work schedule promoting work-life balance.
  • Regular professional development opportunities to enhance your skills and grow in your career.
  • Supportive team environment built on collaboration and mutual respect.
