Key Responsibilities
  • Conduct thorough assessments to tailor treatment plans that meet the unique needs of each patient based on their mental health status and personal goals.
  • Facilitate effective communication among supervisors, direct care staff, and family members by providing updates on individual patient progress and needs.
  • Develop and maintain a detailed schedule of appointments, home visits, and ongoing evaluations to monitor patient advancement in compliance with treatment objectives.
  • Coordinate and execute in-home visits as needed, including chart reviews and staff interviews, to ensure comprehensive understanding of patient circumstances.
  • Analyze and document the effectiveness of program plans, ensuring they are implemented correctly and adjusted as necessary.
  • Actively participate in Interdisciplinary Team Meetings, collaborating with a variety of health professionals to enhance patient care strategies.
  • Complete comprehensive monthly and quarterly progress notes to track patient development and modify treatment plans accordingly.
  • Organize training sessions to ensure that team members are fully competent in implementing program plans and effectively utilizing documentation tools.
  • Provide ongoing support through follow-up interviews, home visits, and observations to foster patient well-being and adherence to treatment goals.
  • Maintain accurate and organized patient records in compliance with healthcare regulations, ensuring that vital information is accessible to relevant stakeholders.
  • Serve as a liaison between the organization and the broader mental health community, helping align development goals with community resources.
  • Assist in the creation and refinement of training materials aimed at enhancing program effectiveness and outcomes for individuals served.
Patient Population / Client Focus

In this role, you will work with a diverse client base, including both adults and youth, addressing a wide range of mental health issues such as anxiety, depression, trauma, and substance misuse. Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
