This position is entirely remote, utilizing telehealth technology to connect with clients in a comfortable and flexible setting. • Conduct thorough assessments to tailor treatment plans that meet the unique needs of each patient based on their mental health status and personal goals.
• Facilitate effective communication among supervisors, direct care staff, and family members by providing updates on individual patient progress and needs.
• Develop and maintain a detailed schedule of appointments, home visits, and ongoing evaluations to monitor patient advancement in compliance with treatment objectives.
• Coordinate and execute in-home visits as needed, including chart reviews and staff interviews, to ensure comprehensive understanding of patient circumstances.
• Analyze and document the effectiveness of program plans, ensuring they are implemented correctly and adjusted as necessary.
• Actively participate in Interdisciplinary Team Meetings, collaborating with a variety of health professionals to enhance patient care strategies.
• Complete comprehensive monthly and quarterly progress notes to track patient development and modify treatment plans accordingly.
• Organize training sessions to ensure that team members are fully competent in implementing program plans and effectively utilizing documentation tools.
• Provide ongoing support through follow-up interviews, home visits, and observations to foster patient well-being and adherence to treatment goals.
• Maintain accurate and organized patient records in compliance with healthcare regulations, ensuring that vital information is accessible to relevant stakeholders.
• Serve as a liaison between the organization and the broader mental health community, helping align development goals with community resources.
• Assist in the creation and refinement of training materials aimed at enhancing program effectiveness and outcomes for individuals served.

In this role, you will work with a diverse client base, including both adults and youth, addressing a wide range of mental health issues such as anxiety, depression, trauma, and substance misuse. Your expertise will empower these individuals to navigate their challenges and achieve their personal growth objectives, positively impacting their daily lives.

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Tennessee is mandatory.
• A minimum of two years of counseling experience in a clinical context is essential.
• A specialization in family therapy, couples therapy, trauma, or substance misuse is highly preferred.
• An in-depth understanding of legal and ethical standards in mental health counseling.
• Exceptional interpersonal and communication skills, with the ability to build rapport across diverse client populations.
• Proven ability to collaborate effectively within a healthcare team to achieve optimal patient outcomes.

• Familiarity with Electronic Medical Records (EMR) software and telehealth technologies is advantageous.
• Prior experience working in a remote setting or delivering services via telehealth is preferred.
