You will engage in regular virtual meetings and collaboratively develop treatment plans that are sensitive to patients' experiences. • Conduct comprehensive mental health evaluations and provide crisis intervention to patients referred to our hospice center, ensuring each individual receives tailored support.
• Collaborate with the Department of Correction to facilitate access to patients, ensuring appropriate housing and treatment plans are in place.
• Coordinate care and develop treatment plans in collaboration with other healthcare providers, particularly focusing on group therapy sessions in mental health observation units.
• Complete required documentation and statistical reports, ensuring compliance with regulatory standards.
• Participate in clinical supervision with the Clinical Supervisor weekly to enhance personal and professional growth.
• Engage actively in mental health and medical rounds, contributing insights and recommendations for patient care.
• Lead case conferences and treatment planning sessions, utilizing a comprehensive approach to address the complex needs of patients.
• Facilitate psychological testing when necessary, and support the supervision of social work interns and volunteers as part of their professional development.
• Coordinate discharge planning with Reentry Social Workers, ensuring patients transition safely and effectively to aftercare services.

The ideal candidate will work with a diverse patient population that includes children, teenagers, and adults facing serious illness or end-of-life issues. Our patients and their families often experience profound emotional distress, and it is crucial that our LCSWs utilize both clinical skills and empathy to guide them through complex emotional landscapes. This role is pivotal in ensuring that patients and families feel supported and understood during difficult times.

• Proven ability to communicate effectively and empathetically with patients and families.

• Possession of a Master’s or Doctorate degree in Social Work, Counseling, Psychology, or a related mental health discipline.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.
• Strong clinical skills with the ability to establish trust and rapport with clients of varying demographics.

• Prior experience providing mental health support in hospice, palliative care, or similar settings.
• Experience working with children and adolescents in therapeutic settings.
• Familiarity with psychological testing and supervised clinical practice.

• Competitive salary ranging from $80,000 to $122,000 per year, commensurate with experience and qualifications.
