Position Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join the Family Relationship Counseling Center team. This full-time remote position plays a vital role in providing psychosocial support to clients receiving hospice care. You'll leverage your expertise in both individual and family therapy, working collaboratively in a multidisciplinary team to ensure holistic, empathetic treatment that improves patient quality of life.

Key Responsibilities
  • Perform comprehensive mental health evaluations, including psychosocial assessments and diagnostic interviews, to identify patient needs accurately.
  • Develop individualized treatment plans in collaboration with patients, setting specific, measurable goals that align with their cultural backgrounds and personal values.
  • Utilize evidence-based therapeutic modalities, such as Cognitive-Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), to facilitate positive outcomes in therapy sessions.
  • Participate in crisis intervention, coordinating timely responses with other healthcare team members to ensure immediate and appropriate care for clients in distress.
  • Accurately maintain clinical documentation, including treatment plans and progress notes, in accordance with regulatory standards and organizational policies.
  • Educate patients and families on mental health strategies and coping mechanisms, empowering them to manage their psychological challenges effectively.
  • Engage with a collaborative care team, including psychiatrists, psychologists, and case managers, to ensure comprehensive approaches to patient care.
  • Conduct outcome evaluations regularly to assess the effectiveness of treatment and modify approaches as needed based on patient feedback and progress.
  • Stay updated on the latest research and best practices in mental health, incorporating new knowledge to enhance your clinical skills.
  • Maintain confidentiality and adhere to ethical guidelines, ensuring the trust and safety of all clients.
Patient Population / Client Focus

This role primarily serves patients experiencing serious illnesses and their families. Our clients often face emotional distress related to grief, terminal diagnoses, and the complex dynamics of end-of-life care. With a strong emphasis on trauma-informed care, you will be prepared to meet the unique emotional and psychological needs of each individual and family, respecting their cultural norms and personal experiences.

Qualifications
  • A Masterโ€™s degree in Social Work is required.
  • A current and active state license as a Licensed Clinical Social Worker (LCSW).
  • Strong ability to work both independently and as part of a multidisciplinary team, demonstrating adaptability in a dynamic environment.
  • Exceptional interpersonal and communication skills with the capacity to build rapport with a diverse patient population.
Preferred Experience
  • Previous experience in hospice or end-of-life care is highly desirable.

Compensation and Benefits
