HowYoullContribute

As a Health Coach,youllsupport patients in our virtual obesity program through ongoing coaching, education, and care coordination.Youll play a key role in driving engagement, behavior change, and overall patient experience - working closely with providers and the care team.

WhatYoullDo

  • Conduct regular virtual check-ins to track progress and motivate patients
  • Support lifestyle, nutrition, and behavior change goals
  • Serve as a primary point of contact for patient questions and support
  • Coordinate with providers, dietitians, and care team members
  • Document patient interactions in the EHR
  • Escalate clinical concerns to licensed providers as needed

WhatWereLooking For

  • 2-5 years of experience in health coaching, patient navigation, care coordination, or a similar patient-facing role.
