JOB SUMMARY:
Reporting to the Clinical Practice and Strategy Partner, the Clinical Health Coach is responsible for providing evidence-based health coaching and clinical guidance to PeopleOne Health clinical members. This role integrates clinical expertise with personalized coaching strategies, focusing on disease management, healthcare navigation, and behavioral modification. The Clinical Health Coach collaborates with healthcare providers, assists with care coordination, and empowers members to adopt sustainable health behaviors through motivational interviewing and personalized coaching plans. The Clinical Health Coach may periodically join team meetings and trainings, as well as appropriate committees and task forces. This position is mainly remote work. Coaching may be a combination of in-person, telephonic, and virtual. Hours vary depending on the client's coaching needs.
SUPERVISORY RESPONSIBILITIES:

ESSENTIAL JOB FUNCTIONS:
  • Provide guidance and support to clinical members, helping them understand and utilize their healthcare benefits and available resources.
  • Conduct health assessments, review medical history, and identify at-risk individuals to provide targeted coaching and support.
  • Track member progress, document sessions, and report outcomes in the electronic medical/health record.
  • Collaborate with the PeopleOne Health Clinical Care Team to create individualized care plans and promote adherence to evidence-based medical recommendations.
  • Guide care by providing education, outreach, options, advocacy, and assistance with navigating the healthcare system.
  • Maintain and follow Clinical Operations protocols, with a focus on chronic disease and condition management, using clinical judgment and discernment.
  • Leverages technology, clinical data, and internal information systems to assist members in accessing optimal and cost-effective healthcare for disease and condition management.

Other Duties:
  • Collaborate with the Marketing Team to develop high-quality, relevant content and resources tailored to member engagement and education (~20% job responsibilities).
  • Responsible for designing and delivering continuing education materials to ensure team members remain informed and proficient across all levels of support.
  • Provide one-on-one coaching, goal setting, and motivational support to individuals, addressing challenges and barriers to health improvement.
  • Act as a resource to the Clinical Operations Team by fostering relationships with healthcare providers, insurers, and wellness program vendors.
  • Work independently to assist individuals in achieving their health and wellness goals through a clinically informed approach.
  • Collaborate with other members of the care team as necessary to ensure a holistic approach to wellness, provide referrals when needed, and integrate multidisciplinary strategies for optimal health outcomes.
  • Stay up to date with the latest research in nutrition, wellness, and coaching methodologies.
  • Create an environment that is supportive, non-judgmental, and conducive to behavior change and health improvement.
  • May be required to work outside of "regular" working hours to accommodate member schedules.
  • May be required to travel to sites within a geographical region or to various locations within a corporately defined area.

  • Strong organizational and time-management skills, ensuring efficient and timely delivery of coaching services. Demonstrated professionalism, ethical conduct, and adherence to member confidentiality.

Requirements
EDUCATION & CERTIFICATIONS :
  • Minimum of 2 years of clinical experience with a Bachelor's degree in a clinically focused health-related field (e.g., Nursing, Dietetics, Public Health, Psychology).
  • Clinical licensure or certification (e.g., RN, RD, mental health professional, or equivalent) preferred.
  • Active certification(s) in fitness, wellness, or related clinical areas (e.g., National Board Certified Health and Wellness Coach, Certified Diabetes Educator, Certified Health Education Specialist) are preferred.

EXPERIENCE:
  • Strong knowledge of the healthcare and insurance industry, including benefits administration and care coordination.
  • Experience in health coaching, patient education, or chronic disease management.
  • Familiarity with electronic medical/health records documentation and telehealth platforms.
