Overview:

Sentara Rockingham Memorial is seeking a Community Health Worker- Outreach for the Hand-in-Hand Resource Mothers Program team.

Virginia Resource Mothers is a home visiting program working specifically with pregnant and parenting teens. Providing them with parenting education, prenatal and postpartum education, and assisting them with resource connection

Works in collaboration with healthcare team members to support diverse populations by facilitating access to essential health services, equipment, and resources that promote health. Provide education on key health topics, support behavior change, screen for risk factors, and partner with community organizations to deliver effective outreach and programming. Help individuals navigate public benefits, connect with support services, and promote positive health outcomes through coaching, education, and early intervention.
This role works with a variety of grant funded programs to bridge the gap between organizations, including health care delivery systems and community health services to: increase culturally relevant knowledge about health and wellbeing; improve underserved populations' access to services; connecting patients to community resources; and ultimately improving health outcomes.
Bilingual in English and Spanish is preferred. Experience working in a community-based setting preferred.
