As a Licensed Clinical Social Worker at Gotham Enterprises Ltd, you will:

• Conduct thorough clinical assessments to determine the psychosocial needs of hospice patients.
• Develop and implement individualized treatment plans employing evidence-based and solution-focused therapeutic interventions.
• Administer therapy sessions, utilizing appropriate methodologies tailored to each patient's needs in a remote setting.
• Maintain accurate clinical documentation in compliance with organizational standards and regulations, enhancing the continuity of care.
• Collaborate with an interdisciplinary team of medical professionals, including doctors, nurses, and chaplains, to ensure coordinated and holistic patient care.
• Participate in case consultations and peer review meetings, fostering an environment of shared knowledge and high-quality care.
• Educate patients and their families on mental health resources and coping strategies, empowering them during their hospice journey.
• Stay abreast of the latest research and developments in mental health and hospice care to enhance patient outcomes.

Your primary focus will be on patients receiving hospice care, including individuals experiencing terminal illnesses, chronic pain, and emotional distress related to end-of-life decisions. You will also provide support to their families, guiding them through the emotional complexities associated with their loved ones' conditions.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Tennessee.
• At least 3 years of clinical experience, with a specific focus on psychotherapy.
• Proficiency in solution-based and evidence-based therapeutic interventions, particularly in remote therapy settings.
• Exceptional communication skills combined with strong analytical and problem-solving abilities.
• Ability to thrive in a collaborative environment, contributing positively to a team-oriented culture.

Experience in hospice care or palliative care settings is highly desirable. Familiarity with electronic health record (EHR) systems and telehealth platforms will enhance your ability to perform in this role effectively.
