• Conduct initial assessments of patients to evaluate their mental health status, focusing on emotional, behavioral, and social functioning.
• Develop and implement personalized treatment plans based on individual needs, integrating evidence-based practices tailored to the unique challenges faced by patients in hospice care.
• Provide psychotherapy to patients coping with a range of mental health issues, including anxiety, depression, grief, and PTSD, ensuring a compassionate and supportive therapy environment.
• Collaborate with a multidisciplinary healthcare team, including physicians and psychiatrists, to create a cohesive approach to patient care, ensuring that physical and mental health needs are addressed simultaneously.
• Regularly monitor patient progress and modify treatment plans as necessary, ensuring responsiveness to patient feedback and updates in their condition.
• Educate patients and their families about mental health conditions, treatment options, and available community resources to empower them in their care process.
• Maintain detailed records of patient interactions and treatment progress using electronic health record (EHR) systems, ensuring compliance with regulatory and ethical standards.
• Continuously adhere to professional ethical guidelines, maintaining patient confidentiality and obtaining informed consent for treatment.

Your role will primarily focus on patients receiving hospice care, who may be grappling with terminal illnesses and associated emotional and psychological challenges. Understanding the complexities of grief, loss, and end-of-life issues will be critical in providing effective support and intervention.

• A Master’s degree in Social Work from an accredited institution.
• Active and current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of 2-3 years of experience providing psychotherapy and mental health services, preferably in a medical or hospice setting.
• Strong clinical skills, specifically in performing mental health assessments, crisis intervention, and treatment plan development.
• Exceptional communication and interpersonal abilities to foster therapeutic relationships with patients and collaborate with diverse healthcare professionals.
• Proficient in using electronic health records (EHR) systems and relevant healthcare software to maintain accurate patient documentation.
• In-depth knowledge of mental health conditions, treatment modalities, and the unique challenges faced by patients in hospice care.
• Commitment to ethical practice and ability to uphold patient confidentiality in a professional setting.

• Experience in hospice care or palliative care settings, with a focus on end-of-life issues.
• Background in working within a multidisciplinary healthcare team.
• Familiarity with telehealth platforms and best practices for remote patient care delivery.

This full-time position offers a competitive salary range of $90,000 to $105,000 annually, commensurate with experience. In addition to salary, we provide comprehensive benefits that include:
