Reliance Youth and Family Counseling Center is seeking a dedicated and Licensed Mental Health Therapist (LMFT, LPC, LCSW) to join our dynamic outpatient team. This position operates primarily in a remote setting, allowing you to provide services to clients from the comfort of your home or preferred workspace. The typical work schedule is Monday through Friday, with hours from 8:00 AM to 5:00 PM. While most sessions will be conducted during these hours, some flexibility is required to accommodate patients' varying needs, which may include occasional evening shifts. You will be embedded in a supportive outpatient office, working closely with an integrated care team to ensure optimal patient outcomes.

• Conduct comprehensive mental health assessments to evaluate the psychological impact of medical conditions on patients’ well-being.
• Develop and implement individualized treatment plans that address both mental health and medical needs, integrating the latest therapeutic approaches.
• Facilitate both individual and group therapy sessions, applying evidence-based strategies to foster resilience and recovery.
• Collaborate actively with medical professionals to coordinate care, ensuring a holistic approach to treatment and recovery.
• Provide immediate crisis intervention and ongoing support during mental health emergencies, leveraging your knowledge and skills to stabilize situations effectively.
• Educate clients about the interplay between mental health and chronic medical conditions, empowering them with strategies to manage their overall health.
• Maintain accurate and confidential patient records in compliance with all legal and ethical guidelines, utilizing electronic health record (EHR) systems as needed.
• Engage in regular team meetings and case conferences to report on client progress and discuss interdisciplinary treatment strategies.
• Stay informed on the latest research and best practices in integrated care, contributing to a culture of continuous improvement.

You will primarily work with individuals experiencing co-occurring mental health and medical disorders, including but not limited to chronic pain syndromes, autoimmune diseases, and substance use disorders. • A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current licensure as an LCSW, LPC, or LMFT in Arizona.
• Demonstrated experience working with patients who have co-occurring medical and mental health conditions.
• Proven ability to collaborate with interdisciplinary teams, ensuring comprehensive and coordinated care.
• Exemplary communication and interpersonal skills, with a strong ability to establish rapport with clients and their families.
• Commitment to ethical practice, maintaining patient confidentiality and professional boundaries.
• Familiarity with EHR systems and the technology necessary for remote counseling.

• A minimum of two years of experience in a mental health counseling role, particularly within integrated care settings.
• Previous experience working with diverse populations, enhancing your ability to provide culturally competent care.
