This position allows for a flexible work schedule, as it is a remote role supported by our hospice center in McDonough, Georgia. • Conduct comprehensive psychological assessments to evaluate patient needs and strengths, tailoring personalized treatment plans accordingly.

• Implement evidence-based therapeutic interventions, including individual counseling and family therapy, to foster emotional healing and resilience.

• Maintain accurate clinical documentation in compliance with state regulations and ethical standards, ensuring patient confidentiality is upheld.

• Collaborate regularly with healthcare providers, nurses, and spiritual counselors as part of a multidisciplinary treatment team to develop integrated care strategies.

• Educate patients and their families about coping mechanisms and resources, ensuring they have the support needed during end-of-life transitions.

• Engage in continuous professional development and stay informed on current research, trends, and best practices in mental health and social work.

Your primary focus will be on hospice patients and their families, addressing mental health challenges associated with terminal illness, grief, and anticipatory loss. • A Master's degree in Social Work from an accredited institution is required.

• A valid LCSW license in the state of Georgia is a must.

• Excellent communication and interpersonal skills to effectively support clients and collaborate with multidisciplinary teams.

• Strong critical thinking and problem-solving skills, essential for case management and intervention strategies.

• Experience with trauma-informed care and grief counseling techniques.

• Proficiency in using electronic health record (EHR) systems and teletherapy platforms.
