This position offers the flexibility of a remote work setting while engaging with a Hospice Center dedicated to compassionate care. As part of our interdisciplinary team, you will interact with fellow social workers, physicians, nurses, and other healthcare professionals, all committed to delivering comprehensive and holistic support to our clients.

• Conduct therapeutic sessions, both individual and group, through secure video conferencing technology, ensuring accessibility and comfort for clients.
• Develop and implement personalized treatment plans based on thorough assessments and ongoing evaluations of clients’ mental health needs.
• Collaborate closely with an interdisciplinary team to create integrated care approaches, sharing insights on client progress and adapting strategies as needed.
• Maintain detailed and accurate client records in compliance with HIPAA regulations, ensuring confidentiality and the integrity of information.
• Provide crisis intervention services as needed, utilizing established protocols to support clients in acute distress.

Your primary focus will be on patients receiving hospice care, a demographic often facing significant life transitions and emotional challenges. You'll have the opportunity to employ trauma-informed approaches, catering to individuals from diverse backgrounds, each with unique stories and needs. Your role will be crucial in helping clients navigate grief, loss, and the complexities of end-of-life care.

• Valid LCSW licensure in the state of New York.
• Bachelor's Degree in Social Work from an accredited institution.
• A minimum of two (2) years of direct experience in a clinical social work setting, preferably in hospice or healthcare environments.
• Exceptional communication and interpersonal skills, with a demonstrated ability to connect with clients from various backgrounds.
• Ability to work effectively in a remote setting, utilizing technology to enhance client interaction.

• Experience with telehealth platforms and virtual therapy methodologies.
• Familiarity with grief counseling and supportive psychotherapy techniques.
• Background in interdisciplinary collaboration within healthcare settings.

Help Wellness Counseling Services offers a competitive salary range of USD 115,000 to 130,000 annually, commensurate with experience and qualifications.
