Key Responsibilities
  • As part of your role reporting to the Social Work Manager, you will assess, diagnose, and treat diverse psychosocial issues impacting patients and their families, employing evidence-based therapeutic modalities.
  • You will conduct comprehensive bio-psychosocial evaluations, eliminating barriers to treatment and enhancing the overall health status of patients.
  • Collaboration is crucial; you will provide necessary psychosocial insights to medical teams, assisting in the development of individualized care plans that align with patient needs.
  • You will facilitate both individual and group therapy sessions, continuously evaluating the effectiveness of interventions and adjusting treatment strategies as required.
  • Documentation is key; you will accurately records all social work activities and patient interactions in medical records in compliance with departmental standards.
  • Educating patients and families is a fundamental aspect of your role, assisting them in understanding treatment recommendations and promoting adherence to care plans.
  • You will actively employ advocacy strategies, connecting patients with community resources that support their social needs and improve their quality of life.
  • Staying updated with available community resources to maximize treatment options will be part of your ongoing responsibilities.
  • Engagement in continuous professional development through relevant training will ensure you maintain your clinical competencies and enhance your expertise.
  • You will uphold adherence to established policies that ensure a safe environment and compliance with regulatory standards.
Patient Population / Client Focus

This role primarily supports patients in hospice care who are dealing with complex psychosocial challenges related to terminal illnesses and end-of-life issues. You will engage with patients who may be experiencing grief, anxiety, depression, and various existential concerns, making it imperative to provide empathetic counseling and a strong support network. Your services will help empower families during one of the most difficult times in their lives, facilitating open dialogue about coping strategies, end-of-life decisions, and emotional resilience.

Qualifications
  • A Masterโ€™s Degree in Social Work from an accredited graduate program.
  • Active Licensed Clinical Social Worker (LCSW) credentials in Tennessee.
  • In-depth understanding of the aging process, human developmental stages, and cultural considerations relevant to care.
  • Minimum one year of clinical experience in health or mental health settings, ideally with populations relevant to hospice or palliative care.
Preferred Experience

Compensation and Benefits

We offer a competitive salary range of USD 85,000 to 100,000 annually, commensurate with experience and qualifications.
