  • Monitor client progress and treatment plans, ensuring timely documentation and effective communication with team members.
  • Participate in quality improvement committees aimed at enhancing the overall effectiveness of service delivery.
Patient Population / Client Focus

You will be working with a diverse client population, including individuals grappling with severe emotional disorders and significant mental health challenges. Given the hospice setting, many clients will be facing end-of-life concerns and transitioning challenges, which will require you to provide not only clinical support but also compassionate, empathetic care. Your ability to establish trust and build rapport with clients from various backgrounds will be crucial to facilitating positive therapeutic outcomes.

Qualifications

  • Must possess a current Licensed Clinical Social Worker (LCSW) credential in the state of Florida.
  • A minimum of 2 years of clinical experience in social work practice, preferably within mental health or hospice settings.
  • Strong interpersonal skills with a proven ability to engage clients, families, and team members in a respectful and culturally sensitive manner.
  • Mature judgment, ethical practice, and compliance with confidentiality regulations and legal standards within social work.
Preferred Experience
  • Experience in a hospice or palliative care setting is a strong plus.
  • Familiarity with integrated care models is advantageous.
  • Proficiency in utilizing case management software and electronic health record (EHR) systems to document and manage client information.
