• Conduct thorough and sensitive assessments of patients and families to identify emotional, social, and spiritual needs.
• Develop personalized treatment plans that respect individual circumstances and preferences while integrating interdisciplinary perspectives.
• Implement evidence-based therapeutic interventions and crisis management strategies to support patients coping with trauma, anxiety, and depression.
• Maintain organized, accurate, and confidential records in compliance with state regulations and organizational policies.
• Engage regularly with multidisciplinary teams, including physicians, nurses, and chaplains, to provide holistic care and ensure cohesive patient support.
• Stay informed on the latest industry research, advancements in social work practices, and hospice care methodologies.

In this role, you will serve a diverse patient population, including individuals facing chronic diseases and their families. Emphasis is placed on providing compassionate support and guidance in navigating the complexities of serious illnesses, from diagnosis through hospice care, ensuring that families have the resources and emotional support they need during this challenging time.

• A Bachelor’s Degree in Social Work from an accredited institution is required.
• An active Licensed Clinical Social Worker (LCSW) license in the state of Georgia is mandatory.
• Strong communication and interpersonal skills to build rapport and trust with patients and their families.
• A solid understanding of ethical practice in social work and the principles of psychosocial assessments and interventions.
• The ability to manage multiple cases effectively, maintain professionalism, and demonstrate empathy in difficult situations.

• Previous experience working in hospice, palliative care, or a similar setting is highly desirable.
• Familiarity with case management software and electronic health record systems.
