• Conduct in-depth psychosocial assessments to collect vital information regarding clients' emotional states, family dynamics, and available support systems, crafting a comprehensive understanding of their unique circumstances.
• Provide empathetic individual, family, and group therapy utilizing evidence-based treatment modalities tailored to meet the specific needs of hospice patients and their families.
• Implement therapeutic approaches, including cognitive-behavioral therapy and solution-focused techniques, to help patients and families navigate the emotional complexities of terminal illness.
• Collaborate with interdisciplinary team members, including physicians, nurses, and spiritual care providers, to develop and execute personalized care plans focused on enhancing quality of life.
• Respond proactively to crisis situations by offering risk assessments, safety planning, and immediate therapeutic interventions as required.
• Facilitate support groups and psychoeducation sessions to empower clients and families, helping them build resilience and coping strategies.
• Maintain precise documentation of all clinical interactions and treatment progress in accordance with ethical and legal standards, ensuring adherence to state regulations.

Our hospice center serves a diverse population of patients, including those facing terminal illnesses such as cancer, heart disease, and neurodegenerative conditions. This role emphasizes a person-centered approach that honors each patient's values, preferences, and life story, providing holistic support to both patients and their families.

• A Master's degree in Social Work from an accredited institution is required.
• Must hold an active state license as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a mental health or outpatient setting, with a preference for experience in hospice or palliative care.
• Strong knowledge of evidence-based therapies and therapeutic modalities appropriate for end-of-life care.
• Excellent interpersonal, communication, and assessment skills, allowing for strong rapport-building with clients and families.

• Experience working in hospice or palliative care settings is highly desirable.
• Familiarity with interdisciplinary team collaboration and the dynamics of end-of-life care.
• Proficiency in electronic health record (EHR) systems to facilitate documentation and patient management.
