Key Responsibilities
  • Conduct thorough assessments for patients and their families, gathering detailed histories to identify psychological, social, and emotional needs.
  • Develop, implement, and evaluate personalized care plans in collaboration with patients, ensuring they align with the holistic goals of hospice care.
  • Utilize evidence-based therapeutic techniques, including cognitive-behavioral therapy (CBT) and supportive counseling, to provide emotional support, crisis intervention, and guidance throughout the caregiving process.
  • Facilitate virtual individual and family therapy sessions aimed at promoting understanding, resilience, and coping strategies during challenging times.
  • Engage with family members through psychoeducation, empowering them with knowledge and resources about grief, loss, and available support systems.
  • Document all interactions, assessments, and treatment plans diligently in our electronic health records (EHR) system, adhering to ethical and legal compliance standards.
