As part of our Hospice Center, you will collaborate with a multidisciplinary team of healthcare professionals, including doctors, nurses, and spiritual advisors, who work together to ensure our clients receive comprehensive and coordinated care.

• Conduct thorough assessments for patients and their families, gathering detailed histories to identify psychological, social, and emotional needs.
• Develop, implement, and evaluate personalized care plans in collaboration with patients, ensuring they align with the holistic goals of hospice care.
• Utilize evidence-based therapeutic techniques, including cognitive-behavioral therapy (CBT) and supportive counseling, to provide emotional support, crisis intervention, and guidance throughout the caregiving process.
• Facilitate virtual individual and family therapy sessions aimed at promoting understanding, resilience, and coping strategies during challenging times.
• Engage with family members through psychoeducation, empowering them with knowledge and resources about grief, loss, and available support systems.
• Document all interactions, assessments, and treatment plans diligently in our electronic health records (EHR) system, adhering to ethical and legal compliance standards.
• Attend and actively participate in team meetings and case conferences to share insights, discuss patient progress, and foster collaboration within the care team.

The primary clientele includes individuals experiencing terminal illnesses and their families. Your work will significantly impact families navigating complex emotions related to end-of-life issues. You will provide essential emotional and spiritual support, ensuring that patients feel valued and understood during this critical phase of life.

• Master's degree in Social Work (MSW) from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in Texas.
• Minimum of two years of experience in social work, particularly in hospice, palliative care, or similar settings.
• Strong proficiency in mental health assessment tools and therapeutic interventions.
• Knowledge and application of trauma-informed care principles and understanding of grief and bereavement issues.
• Excellent interpersonal abilities and strong communication skills, facilitating trust and meaningful connections with clients and their families.

• Experience in hospice or palliative care settings highly preferred.
• Familiarity with patient management software and EHR systems.
• Proven track record of working effectively within multidisciplinary teams.
• Advanced training in grief counseling and bereavement support.

We offer a competitive salary range of $80,000 to $100,000 per year, commensurate with experience and qualifications.
