Mohawk Trail Counseling Center is seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join our growing team in a fully remote capacity. This role is designed for a remote work setting, allowing flexibility while maintaining the valued connection with our patients and the primary care team. You will have access to digital communication tools to facilitate patient interactions and provide therapy through telehealth platforms while adhering to HIPAA guidelines. As an LPC at Mohawk Trail Counseling Center, your responsibilities will include:

• Collaborating with primary care providers to recognize, treat, and manage mental health conditions and psychosocial concerns, contributing significantly to the patient care team.
• Conducting comprehensive patient intakes and evaluations, focusing on psychological diagnostics and functional assessments to formulate personalized treatment plans.
• Providing brief therapeutic interventions and ongoing support for patients experiencing mental health challenges, tailored to their specific needs.
• Advising on treatment pathways, discerning whether patients can be adequately supported within the primary care setting or require referrals to specialized services.
• Conducting follow-up consultations to assess patient progress, adjusting treatment plans as necessary, and developing relapse prevention strategies.
• Identifying at-risk patients and devising care plans to mitigate potential crises and enhance mental health outcomes.
• Documenting patient progress accurately in treatment charts and keeping primary care providers informed of treatment developments and recommendations for continuity of care.
• Fostering patients’ self-management skills through education and resources to empower them in their own recovery journeys.
• Playing a consultative role in helping the clinic’s management and staff understand and integrate behavioral health services effectively.
• Adhering to the Code of Ethical Conduct for Clinical Professionals and performing additional duties as assigned.

Your work will primarily involve adult patients who seek support for a range of mental health issues, including anxiety, depression, trauma, and behavioral concerns. Many of our patients face social determinants of health, necessitating a nuanced and empathetic approach. By promoting an environment of trust and confidentiality, you will assist clients in navigating both their mental health challenges and the broader implications for their families and communities.

The ideal candidate will possess:

• A Master's Degree in Clinical Counseling.
• A current LPC (Licensed Professional Counselor) license in the state of Oregon.
• A solid understanding of mental health assessment techniques, treatment planning, and psychotherapeutic modalities, including knowledge of DSM-IV diagnoses.
• Excellent communication skills and the ability to build rapport with a diverse patient population.
• Capacity to work effectively within an interdisciplinary team and establish cooperative relationships with both patients and healthcare colleagues.

While not mandatory, the following qualifications are preferred:

• Bilingual proficiency in English and Spanish to better serve our diverse community.
• Experience working within a Federally Qualified Health Center (FQHC) and knowledge of the complexities of community health resources.
• A demonstrated ability to employ psychotherapeutic techniques and behavioral change strategies effectively.

Mohawk Trail Counseling Center offers a competitive salary in the range of USD 87,000 – 105,000 annually, commensurate with experience.
