Key Responsibilities
  • Conduct comprehensive assessments to identify the needs and goals of patients, ensuring that treatment plans are appropriately tailored.
  • Engage in ongoing communication with supervisors, coworkers, and family members regarding client progress, utilizing a collaborative approach for optimal outcomes.
  • Develop and maintain a tentative schedule that accounts for patient appointments, home visits, and observations to monitor care progress.
  • Coordinate and perform essential home visits to conduct in-home assessments, chart reviews, and interviews, guaranteeing responsive and locally relevant care.
  • Oversee the effective implementation of individualized program plans, continually evaluating their success and making adjustments as needed.
  • Participate actively in Interdisciplinary Team Meetings and contribute insights that enhance treatment quality and team dynamics.
  • Document monthly and quarterly progress notes in accordance with best practices, ensuring that all developments are accurately recorded.
  • Organize and execute training for team members to enhance their competencies in implementing treatment plans and maintaining necessary documentation.
  • Provide follow-up support through interviews, home visits, and observational assessments to ensure the ongoing success of treatment interventions.
  • Act as a liaison with the broader mental health community, fostering relationships that enhance program development and public support.
Patient Population / Client Focus

Your work will encompass a diverse client base, including both adults and youth experiencing various mental health challenges. You will utilize your expertise to address a range of issues, including family dynamics, substance misuse, and trauma, tailoring your approach to meet individual needs and promote healing.

Qualifications
  • A Masterโ€™s or Doctoral degree in Counseling, Psychology, or a related field is required.
  • Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Kansas is mandatory.
  • A minimum of 2 years of clinical counseling experience is essential, demonstrating your capability to engage effectively with patients.
  • Specialization in areas such as family therapy, couples therapy, trauma, or substance misuse is highly advantageous.
