The Wellbeing Counseling Center is seeking a dedicated and compassionate Remote Licensed Clinical Professional Counselor (LCPC) to join our team on a full-time basis. You will primarily serve patients in an outpatient capacity, which may include conducting video counseling sessions, participating in team meetings, and coordinating with other health professionals. Key Responsibilities
  • Conduct thorough assessments to understand the unique needs and circumstances of each client, tailoring treatment plans accordingly.
  • Maintain regular communication with supervisors, direct care staff, and family members to discuss client progress and address any emerging challenges.
  • Develop and provide a tentative schedule of appointments, home visits, and observations to ensure effective monitoring of clients' advancement.
  • Coordinate necessary home visits to conduct observations, chart reviews, and interviews to gather relevant data for treatment planning.
  • Ensure the effective implementation of program plans, regularly reviewing and adjusting strategies as necessary to promote client success.
  • Participate actively in Interdisciplinary Team Meetings and other relevant discussions to foster a holistic approach to care.
  • Document client progress through monthly and quarterly notes, ensuring compliance and accessibility of information for authorized stakeholders.
  • Facilitate training sessions for staff and team members to enhance their competencies in implementing program plans and maintaining up-to-date documentation practices.
  • Provide ongoing support through follow-up interviews and home visits to address any persistent or evolving needs of clients.
  • Serve as a liaison between the center and the wider community to strengthen support networks and enhance recovery avenues for clients.
  • Contribute to the development of training materials and procedural guidelines to enrich the quality of care we deliver.
Patient Population / Client Focus

Your work will involve engaging with a wide range of clients, including adults and youth, who are seeking help for various mental health challenges such as depression, anxiety, trauma, substance misuse, and relationship issues. Qualifications
