This position is structured as a full-time remote role, allowing you the flexibility to provide compassionate care from your chosen location. You will work closely with a team of healthcare professionals, including psychiatrists, psychologists, and nursing staff at the hospice center, ensuring seamless collaboration and communication to effectively meet each patient's needs. Your home office will be equipped with the necessary tools and technology to facilitate client interactions, teletherapy sessions, and maintaining documentation.

• Conduct in-depth psychosocial assessments to identify clients’ mental health concerns, strengths, and available resources.
• Deliver individual, family, and group therapeutic sessions, employing evidence-based treatment modalities tailored to each client's needs.
• Design and implement individualized treatment plans with clear goals and interventions, regularly reviewing and updating these plans as needed.
• Collaborate with interdisciplinary team members to ensure comprehensive, coordinated care that addresses the psychological, emotional, and physical needs of clients.
• Facilitate crisis intervention, conducting risk assessments and developing safety plans for clients in acute distress.
• Maintain accurate and timely clinical documentation in accordance with ethical and legal standards, ensuring compliance and quality assurance.
• Lead support group sessions, psychoeducation programs, and workshops to enhance community engagement and support.

Your primary focus will be on patients in hospice care, a population facing unique emotional and psychological challenges. You will support not only the patients but also their families, providing guidance and coping strategies as they navigate complex emotions and decisions related to end-of-life care. • A Master’s degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Minimum of three years of experience in the mental health field, particularly in outpatient and hospice care settings.
• Demonstrated knowledge of evidence-based therapeutic modalities, including cognitive behavioral therapy, trauma-informed care, and solution-focused therapy.
• Exceptional interpersonal, communication, and assessment skills are essential for effectively engaging with clients and their families.

• Experience in crisis intervention and risk assessment involving patients with severe mental health issues.
• Familiarity with telehealth platforms and electronic health record (EHR) systems.
• Background in working with diverse populations and cultural competency in addressing varying mental health needs.
