• Conduct thorough clinical assessments to evaluate the emotional and psychological needs of patients in hospice care.

• Develop and implement individualized treatment plans that address the unique challenges faced by patients and their families.

• Utilize solution-focused therapeutic interventions to help patients cope with grief, loss, and other emotional difficulties.

• Maintain accurate and timely clinical documentation to ensure compliance with regulations and facilitate effective treatment coordination.

• Collaborate with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to deliver holistic and integrated care.

• Offer education and support to patients and families regarding end-of-life options, coping strategies, and available resources.

• Stay abreast of current mental health research and trends to continually enhance patient care practices.

This position primarily focuses on providing care for patients within a hospice setting, supporting individuals and families who are facing terminal illnesses. You will work with a diverse group of clients, presenting a range of emotional and psychological challenges, and will have the opportunity to create lasting relationships while guiding them through their unique journeys. • A Master’s degree in Social Work from an accredited institution.

• A valid LCSW license in the state of Arizona is required.

• Experience working in a hospice or palliative care setting is highly desirable.

• Familiarity with electronic health record (EHR) systems and patient management software.

• The annual salary for this position ranges from $80,000 to $104,000, commensurate with experience and qualifications.
