Work Environment

This role is primarily conducted in a remote setting, allowing for a flexible work schedule that can accommodate your personal and professional needs. Key Responsibilities
  • Conduct comprehensive assessments to understand and address the unique needs of each patient, implementing tailored treatment plans.
  • Facilitate clear communication among supervisors, direct care staff, family members, and other stakeholders to track patient progress effectively.
  • Establish and maintain a tentative schedule for client appointments, home visits, and essential evaluations, balancing caseload effectively.
  • Conduct home visits when necessary for in-home observations, patient assessment, and data collection.
  • Ensure that the treatment plan is executed effectively, adjusting as needed based on ongoing assessments and feedback.
  • Regularly participate in Interdisciplinary Team Meetings and other relevant discussions to review patient care and program effectiveness.
  • Document and submit monthly and quarterly progress notes to maintain accurate records, ensuring compliance with legal and ethical standards.
  • Plan and organize training sessions for staff to enhance understanding and implementation of program protocols and documentation requirements.
  • Provide continued support to patients through follow-up interviews and observations, ensuring their evolving needs are met.
  • Act as a liaison with the public and the mental health community to promote services, goals, and community engagement initiatives.
  • Assist in the development and refinement of training materials to improve program quality and documentation processes.
Patient Population / Client Focus

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
