Work Environment

This position supports a community health clinic setting while providing remote therapy solutions, allowing flexibility to manage your schedule effectively. The work hours for this role are Monday to Friday, from 8:00 AM to 4:30 PM, with occasional requirements for evening or weekend hours based on client needs. The role requires adaptability to accommodate visits and ensure continuous support for our senior clients, including on-call availability for urgent situations.

Key Responsibilities
  • Conduct comprehensive assessments of seniors' psychosocial needs by evaluating emotional, mental, and social factors influencing their wellbeing.
  • Collaborate intimately with clients and their families to develop personalized care plans that directly address individual goals, preferences, and challenges.
  • Provide counseling and emotional support, empowering seniors through transitional phases and daily challenges in their life.
  • Advocate for clients' rights within the healthcare system, guiding them in accessing community resources and ensuring they receive appropriate support services.
  • Work cooperatively with an interdisciplinary team, including healthcare professionals, nurses, and caregivers, to foster coordinated care for your clients.
  • Perform regular home visits or virtual check-ins, monitoring the well-being of clients and addressing any safety concerns or living condition issues that may arise.
  • Facilitate family communication, helping to resolve conflicts and enhance the overall support network surrounding each client.
  • Educate clients and their families about available community resources, support groups, and programs to promote social engagement and improve their quality of life.

Compensation and Benefits
