At Emotional Restoration Counseling Services, we are seeking a remote, full-time LCSW to provide high-quality psychosocial support to our clients from the comfort of your home. This role is critical to enhancing the emotional well-being of patients receiving care through our hospice center located in beautifully scenic Bend, Oregon.

Key Responsibilities
  • Provide compassionate care by conducting comprehensive assessments that evaluate clientsโ€™ emotional and mental health needs.
  • Develop, implement, and periodically revise individualized treatment plans to assist clients in achieving their mental health goals.
  • Utilize established therapeutic practices to address a range of mental health conditions while integrating client and family feedback into ongoing treatment delivery.
  • Document all interactions, treatment plans, and progress notes accurately in our electronic health record (EHR) system to ensure continuity of care.
  • Work collaboratively with our dedicated team of healthcare professionals to coordinate care and support for each client, contributing to multidisciplinary team meetings and case reviews.
  • Remain current with best practices in social work and hospice care, ensuring services provided are evidence-based and aligned with the latest mental health guidelines.
  • Engage in case management duties, providing clients with referrals to additional resources and support services as needed.
  • Participate in quality assurance meetings to help us continually improve our service delivery and patient care.
Patient Population / Client Focus

In this role, you will primarily support individuals and families facing the emotional and psychological challenges that arise from serious health issues and the hospice process. You will work with a diverse patient population, including clients from various age groups, backgrounds, and experiencesโ€”including those navigating grief, illness, and end-of-life transitions. Qualifications
  • Masterโ€™s degree in Social Work (MSW) with a valid Licensed Clinical Social Worker (LCSW) license in the state of Oregon.
