• Provide comprehensive social work services to clients through virtual consultations, ensuring they receive personalized support aligned with their unique needs.

• Conduct thorough assessments of clients' psychosocial needs, developing effective treatment plans that address emotional, social, and practical concerns specific to hospice care.

• Advocate for clients' rights and access to essential resources, navigating complex healthcare systems to facilitate appropriate care and support services.

• Collaborate with interdisciplinary teams, including nurses, physicians, and chaplains, to ensure holistic care that integrates medical treatment and emotional support.

• Organize and lead group support sessions for patients and families, providing a space to express thoughts and feelings related to grief and loss.

• Maintain accurate and timely documentation of client interactions and treatments in compliance with relevant regulations and organizational policies.

This role centers on providing emotional and practical support to terminally ill patients and their families. The hospice environment presents unique challenges, such as addressing end-of-life issues and preparing family members for significant emotional transitions. You will play a critical role in helping families navigate the complexities of grief while ensuring that patients receive dignified and respectful care throughout their final moments.

• Master's degree in Social Work (MSW) from an accredited institution.

• Licensed Clinical Social Worker (LCSW) in the state of California, demonstrating a commitment to professional standards and ethical practice.

• A minimum of three (3) years of experience in social work, with a preferential focus on hospice or palliative care settings.

• Exceptional skills in communication, empathy, and crisis intervention, with the ability to work effectively with individuals from diverse backgrounds.

• Proficient in using remote work technology, including telehealth platforms, to deliver effective services.

• Familiarity with hospice care models and end-of-life issues is highly desirable.

• Experience in conducting bereavement counseling or support group facilitation.

• Previous involvement with interdisciplinary healthcare teams, showcasing an ability to collaborate across various roles.

This full-time position offers a competitive salary ranging from $78,000 to $115,000 annually, commensurate with experience and qualifications.
