This role is primarily remote, allowing you to engage with clients and collaborate with our interdisciplinary team from the comfort of your own home. While most interactions will be virtual, occasional on-site visits may be necessary to provide client assessments and foster direct engagement. • Conduct thorough psychosocial assessments to evaluate the emotional, mental, and social needs of our senior clients.
• Collaborate with seniors and their families to develop customized care plans that align with their personal goals and preferences.
• Provide individual and family counseling, assisting clients in navigating transitions, such as moving into assisted living or dealing with loss.
• Advocate for clients, ensuring they have access to necessary community resources and services that support their health and independence.
• Engage in regular, proactive client monitoring visits to assess living conditions, address safety concerns, and ensure ongoing well-being.
• Foster open communication amongst family members involved in the clients' care, mediating conflicts when necessary to promote harmony.
• Educate clients and families about available community resources, support groups, and activities that can enhance social interaction and life satisfaction.
• Maintain accurate, confidential records of client interactions, assessments, and service plans using our electronic health record system.
• Stay up to date with current social work best practices, policies, and ethical standards relevant to geriatric care.

Your primary focus will be on seniors, who often face complex psychosocial challenges that require tailored interventions. By understanding their unique life experiences and needs, you will play an essential role in enhancing their quality of life and emotional resilience.

• Master's degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in New York State.
• A minimum of three years of clinical social work experience, preferably in community health settings focusing on geriatric populations.
• Strong knowledge of aging-related issues, community resources, and social work principles.
• Excellent assessment, counseling, and intervention skills tailored to seniors and their families.
• Demonstrated ability to empathize and build rapport with clients facing significant life changes.
• Exceptional communication and interpersonal skills for effective collaboration with multidisciplinary teams.
• Proficiency with electronic health records (EHR) systems and remote communication tools.

• Experience in advocacy and navigating complex social service systems.
• Background in geriatric social work or related fields.
• Demonstrated capability in crisis intervention and conflict resolution.

The salary range for this position is $80,000 to $100,000 per year, commensurate with experience.
