As a remote Licensed Clinical Social Worker, you will engage with clients through virtual sessions, bringing care right to their living spaces. • Conduct comprehensive assessments to identify the emotional, social, and medical needs of patients entering hospice care.
• Develop tailored care plans in collaboration with interdisciplinary teams, including physicians, nurses, and family members, ensuring a holistic approach to patient care.
• Provide counseling and support to patients and their loved ones, guiding them through the emotional challenges associated with terminal diagnoses.
• Facilitate family meetings to discuss care goals, address concerns, and ensure alignment with patient wishes.
• Monitor and evaluate the effectiveness of interventions, adjusting care plans as necessary to maximize patient comfort and quality of life.
• Document all patient interactions, assessments, and treatment plans accurately, adhering to organizational policies and compliance standards.

As a Licensed Clinical Social Worker at Gotham Enterprises Ltd, you will primarily work with patients facing serious, life-limiting illnesses and their families. • A minimum of a Bachelor's Degree in Social Work (BSW) from an accredited institution.
• Current and active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Missouri.

• Strong interpersonal skills, with the ability to build rapport and trust with patients and families.
• Excellent communication skills, both written and verbal, to effectively convey information and facilitate meaningful conversations.

• Previous experience working in a hospice or palliative care environment is highly desirable.
• Experience in crisis intervention and grief support.
• Familiarity with telehealth and virtual counseling platforms.
• Ability to work independently while maintaining a collaborative spirit with the interdisciplinary team.
