• Conduct comprehensive assessments by interviewing clients and evaluating their current emotional, psychological, and social conditions, thereby identifying their specific needs, strengths, and weaknesses.
• Develop and implement personalized treatment plans tailored to each client's individual circumstances, setting measurable goals to track their progress.
• Monitor client progress consistently and adjust plans as required to ensure effective treatment outcomes.
• Facilitate counseling sessions, providing clients with informed guidance and strategies to tackle their challenges effectively.
• Keep detailed and accurate records of client interactions, treatment progress, and actionable insights to uphold compliance and facilitate case management.
• Participate in interdisciplinary team meetings, sharing insights and contributing to comprehensive discussions on client care.
• Act as a liaison between clients and community resources, providing referrals as necessary to enhance recovery and ensure clients have access to a full spectrum of support services.
• Prepare and present client cases during case conferences and assist in providing testimony as needed in court settings.

In this role, you will work with a diverse array of clients facing a wide range of mental health challenges, including anxiety, depression, substance use disorders, and trauma-related issues. Your work will primarily focus on adults within the community, providing them with essential coping strategies and emotional support.

• A Master’s degree in Social Work (MSW) from an accredited institution, which emphasizes clinical practice.
• Current and valid LCSW license to practice in the state of Georgia.
• Demonstrated experience as a Social Worker, showcasing a strong understanding of social work theories and practices.
• Exceptional communication skills, with the ability to build rapport and maintain professional relationships across diverse populations.
• A high degree of empathy and social perceptiveness, enabling effective responses to client needs.

• Previous experience within community health settings or with multidisciplinary teams is highly desirable.
• Familiarity with telehealth tools and virtual communication platforms will be beneficial.
• Background in crisis intervention or trauma-informed care approaches.
