• Conduct thorough mental health assessments to evaluate the psychosocial status of patients, including detailed diagnostic interviews and comprehensive risk assessments to inform treatment plans.
• Collaborate with patients, families, and the multidisciplinary team to create individualized treatment plans that are culturally aware and tailored to meet the diverse needs of each patient.
• Utilize a range of evidence-based therapeutic interventions such as CBT, DBT, and SFBT across various settings, including individual, couple, family, and group therapy sessions.
• Provide crisis intervention services, ensuring that patients in acute distress receive immediate support and coordination of care with other health professionals.
• Maintain detailed clinical documentation including progress notes, treatment plans, and timely discharge summaries following established policies and regulatory frameworks.
• Educate patients and their families through psychoeducation, empowering them to better understand and manage mental health conditions.
• Work closely with a multidisciplinary team including psychiatrists, psychologists, and case managers to ensure a cohesive and comprehensive approach to patient care.
• Regularly assess treatment outcomes and adjust care strategies based on ongoing evaluations and patient feedback.
• Stay updated on the latest treatment methodologies and mental health trends to continually enhance clinical practice.
• Uphold the highest ethical standards and confidentiality in all patient interactions and documentation.

• A Master's degree in Social Work (MSW) is essential for this position.
• An active state license as a Licensed Clinical Social Worker (LCSW) is required.

Experience in hospice or palliative care settings is highly preferred.
