• Conduct comprehensive psychosocial assessments for patients diagnosed with cancer, identifying their support systems, coping strategies, and community resources to best meet their needs.
• Deliver crisis intervention and short-term counseling via teletherapy to patients and families dealing with acute distress, ensuring they receive immediate, empathetic support during critical times.
• Facilitate ongoing counseling sessions, guiding patients through the continuum of their illness while minimizing stress for both them and their families.
• Educate patients and family members about available community resources and coping mechanisms tailored to their unique circumstances.
• Collaborate with interdisciplinary team members to contribute to patient care plans and ensure comprehensive support for patients receiving care in a hospice setting.
• Demonstrate adherence to organizational compliance policies and the Code of Conduct, proactively addressing any compliance concerns and reporting violations as necessary.

The primary focus for this role will be on cancer patients and their families. You will engage with individuals facing the emotional and psychological impact of serious illness, including end-of-life considerations. This requires a sensitive approach, as many patients are experiencing significant distress, grief, and anticipatory anxiety. Understanding their unique narratives and providing tailored support is pivotal for optimizing their quality of life.

• A Master’s degree in Social Work from an accredited institution is required.
• An active and valid LCSW license in the state of California is essential to practice.
• A minimum of 2 years of professional experience as a social worker, with a focus on healthcare or hospice settings preferred.
• Exceptional clinical assessment and diagnostic skills are crucial for identifying and addressing patients' psychosocial needs.
• Strong written and verbal communication abilities to effectively engage with patients, families, and multidisciplinary team members.
• A demonstrated ability to work both independently and collaboratively in a remote setting.

• Experience with teletherapy and virtual patient interactions is highly beneficial.
• Previous work in a hospice, oncology, or palliative care environment would be a distinct advantage.
