Key Responsibilities
  • Conduct comprehensive assessments to determine the counseling needs of clients, including substance abuse evaluations using DSM criteria for accurate diagnosis.
  • Prepare and implement individualized treatment plans that incorporate counseling interventions for clients diagnosed with substance use disorders.
  • Develop discharge and continuity of care plans that facilitate clientsโ€™ transition to the most appropriate level of care, ensuring ongoing support and resources.
  • Document all interactions, assessments, treatment plans, and progress notes in clientsโ€™ clinical records accurately and timely.
  • Maintain a manageable caseload, providing case management services and adhering to treatment planning and clinical charting standards.
  • Coordinate support services for clients and their families, documenting all provided services in clinical records.
  • Engage with referral sources, including legal entities, educational institutions, and employers, to provide comprehensive care for clients.
  • Facilitate crisis interventions as necessary, employing a collaborative multi-disciplinary team approach to address urgent client needs.

Qualifications
