About the Organization

Our multidisciplinary team excels in diagnosing, treating, and managing a comprehensive range of mental health conditions such as depression, anxiety, and trauma-related disorders. We pride ourselves on creating a supportive environment that prioritizes healing and personal growth for clients of all ages.

Work Environment

This role provides the flexibility of a remote work setting, allowing you to connect with clients via secure telehealth platforms. Openness to working some evenings and weekends is essential, as we strive to accommodate the varied schedules of our clients. You will also interact with our team of dedicated professionals to engage in collaborative care, ensuring all clients receive compassionate and comprehensive support tailored to their unique needs.

Key Responsibilities
  • Conduct thorough psychosocial assessments to understand the mental health concerns, strengths, and resources of clients, allowing for tailored treatment plans.
  • Provide individual and family therapy sessions, employing evidence-based treatment modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care.
  • Collaborate with a multidisciplinary treatment team, including psychiatrists and psychologists, to ensure coordinated care is in place for clients.
  • Develop and implement individualized treatment plans with specific, measurable goals and interventions tailored to each client's needs.
  • Offer crisis intervention, risk assessment, and safety planning during times of acute need, ensuring client welfare is prioritized at all times.

Many of your clients will be dealing with complex emotions related to hospice care, making it essential that you possess a high degree of empathy, understanding, and professionalism in your approach. Your role is vital in helping to navigate these sensitive issues, ultimately aiming to enhance their quality of life.

Qualifications
  • A Master's degree in Social Work from an accredited program.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
  • Strong foundational knowledge of various evidence-based therapeutic modalities and treatment processes.
  • Exceptional interpersonal, communication, and assessment skills to engage effectively with clients and the treatment team.
Preferred Experience

While minimum experience requirements include at least two years in a mental health or outpatient setting, candidates with specific experience working with hospice clients or end-of-life issues will be given preference.
