Key Responsibilities
  • Develop, implement, and evaluate comprehensive clinical treatment plans for Health Plan members, including diagnosis, treatment, and referrals for those experiencing acute or chronic mental health issues.
  • Engage in collaborative staff conferences to review and assess treatment strategies and adjustments for optimal client care.
  • Provide outpatient psychotherapy, utilizing telehealth platforms, to individuals, couples, families, and groups tailored to senior clientsโ€™ unique needs.
  • Accurately prepare intake summaries, treatment progress notes, and ongoing case summaries while ensuring compliance with state regulations and NCQA standards.
  • Maintain detailed and confidential records of each client's treatment journey, ensuring a respectful and ethical approach to their care.
  • Participate in an on-call rotation if required, to support the clinic's commitment to accessible mental health services.
  • Evaluate the needs of elderly clients by listening empathetically and developing customized strategies to enhance their mental health and overall well-being.
  • Research available community resources and provide referrals to clients to support their therapy and recovery goals.
Patient Population / Client Focus

This role primarily focuses on serving our senior clients, who may be facing a range of psychological and emotional challenges. You will have the opportunity to develop deep relationships with your clients, understanding their unique experiences and providing support as they navigate the complexities of aging, health issues, loss, and relationship changes.

Qualifications
  • A Masterโ€™s degree in Social Work, Clinical or Counseling Psychology, or a related field from an accredited institution.
  • Valid professional license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or equivalent in California, as associate licenses are not billable through Medicare.
  • Ability to work independently with minimal supervision, demonstrating self-direction in clinical practices.
  • Strong proficiency in using technology and software systems, including Microsoft Office Suite.
  • Capability to thrive in a fast-paced environment while managing multiple clients with diverse needs effectively.
Preferred Experience
  • Experience working with the geriatric population is preferred but not mandatory, as we value a wide range of clinical backgrounds.
  • Previous exposure to telehealth practices and electronic health records (EHR) systems.
  • Familiarity with community resource navigation and client advocacy.
