• Conduct comprehensive mental health assessments that account for underlying medical conditions impacting patient mental well-being.
• Design and execute integrated treatment plans, incorporating various evidence-based therapeutic modalities to support individuals in achieving their treatment goals.
• Facilitate both individual and group therapy sessions, adapting therapeutic approaches to fit the specific needs and situations of patients.
• Collaborate with a team of medical professionals to deliver coordinated care that enhances treatment efficacy and patient satisfaction.
• Provide crisis intervention and immediate support during mental health emergencies, ensuring the safety and welfare of all patients.
• Educate patients about the interplay between mental health and physical health, promoting strategies for self-management of their conditions.
• Maintain accurate, confidential documentation in compliance with all legal and professional standards, using electronic health record systems.
• Engage actively in team meetings and case conferences, contributing your insights to interdisciplinary care planning.
• Stay informed of current research and evolving best practices in integrated mental health care.

The patients you will support include individuals facing complex mental health challenges compounded by medical conditions such as autoimmune disorders, chronic pain, and substance use disorders. Your expertise will be essential in helping these patients navigate their diverse mental health needs while collaborating closely with their other healthcare providers to offer a comprehensive treatment experience.

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Currently licensed as an LCSW, LMHC, or LMFT in the state of New York.
• Demonstrated experience in addressing mental health issues within the context of co-occurring medical conditions.
• Exceptional interpersonal and communication skills that facilitate effective patient interactions.
• Strong ethical judgement to maintain professional boundaries and patient confidentiality.
• Proficiency with electronic health records and telehealth technology.
• An ability to work collaboratively as part of an interdisciplinary team, contributing to shared strategies for patient success.

• Previous experience in a psychiatric outpatient setting focusing on integrated health care delivery.
• Familiarity with evidence-based practices specific to mental health therapy.
• Experience in crisis intervention and risk assessment in fast-paced environments.
