• Conduct comprehensive evaluations and assessments to determine patients' needs, developing personalized treatment plans accordingly.
• Facilitate effective communication among multidisciplinary teams, family members, and other stakeholders to share patient progress and ensure cohesive care delivery.
• Document and track ongoing treatment progress, including scheduling of appointments, home visits, and observation sessions to provide holistic care.
• Organize and participate in home visits to conduct assessments and provide support tailored to individual patient circumstances.
• Maintain compliance with program strategies, ensuring they are executed effectively and documented properly.
• Engage in interdisciplinary team meetings, contributing insights and updates on patient care plans.
• Complete monthly and quarterly progression reports to monitor effectiveness and outcomes of treatment strategies.
• Arrange and conduct training sessions for staff ensuring proficiency in program execution and documentation requirements.
• Follow up with patients through interviews and observations to continuously meet their evolving needs.
• Manage and oversee patient records, ensuring compliance with documentation standards and accessibility for necessary stakeholders.
• Serve as a vital connection between our community and the mental health care system, assisting clients in achieving their personal development goals.
• Participate in creating materials and strategies aimed at enhancing individual programs and staff training.

As a Licensed Professional Counselor at Brave Mind Mental Clinic, you will work with a diverse client population, including adults and youth facing various mental health challenges. Our clients often deal with issues ranging from anxiety and depression to trauma and substance misuse, emphasizing the need for personalized and empathetic care tailored to each individual’s life circumstances.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is essential.
• An active and unrestricted license as a Professional Counselor (LPC) in the state of Oregon is required.
• A minimum of two years of counseling experience, ideally in a clinical context, is necessary.
• Experience or specialization in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is highly preferred.
• Strong knowledge of legal and ethical standards related to mental health practices.
• Exceptional interpersonal and communication skills, with a demonstrated ability to build rapport with diverse clients.
• Ability to work effectively in a collaborative healthcare team environment.
• Proficiency in electronic medical records (EMR) software and relevant technological tools.

While a minimum of two years of counseling experience is required, candidates with a strong background in specialized therapeutic practices such as cognitive behavioral therapy, dialectical behavior therapy, or trauma-informed care would be ideal.
