Work Environment

This position is entirely remote, offering you the convenience to work from anywhere while still being a vital part of our outpatient office. You will utilize a variety of digital tools and electronic medical record (EMR) systems to provide effective care management and documentation. Your days will be structured to allow for individual client counseling sessions, team meetings, and collaborative care efforts, all within a supportive atmosphere that emphasizes work-life balance.

Key Responsibilities
  • Conduct thorough and ongoing assessments to identify the needs and goals of patients, ensuring tailored treatment plans are developed and implemented.
  • Communicate effectively with a range of stakeholders, including supervisors, direct care staff, family members, and caregivers, to discuss and monitor individual progress.
  • Prepare and maintain a schedule of appointments, home visits, and observations to track patients’ advancements and ensure continuous progress in their treatment.
  • Coordinate and conduct home visits to fulfill the requirements of in-home assessments, ensuring comprehensive support through direct observation and data collection.
  • Oversee the implementation of program plans, ensuring that all protocols are followed, and contribute to interdisciplinary meetings as needed to discuss patient care strategies.
  • Document monthly and quarterly progress notes that reflect patient developments, treatment modifications, and any other relevant information.
  • Organize training sessions aimed at certifying staff in the application of program plans and the proper utilization of documentation processes.
  • Facilitate follow-up interviews and home visits to offer ongoing support and ensure that the evolving needs of clients are met effectively.
  • Maintain accurate and accessible records for interested parties while acting as a liaison with the broader mental health community to enhance collaborative efforts and development goals.
  • Assist in crafting procedures for training materials that elevate the quality of programs provided to individuals.
Patient Population / Client Focus

Our LPC will work with a diverse client base consisting of both adults and youth across various backgrounds, addressing a range of mental health challenges, including anxiety, depression, trauma, and substance misuse. The primary focus is on fostering therapeutic relationships that pave the way for healing, growth, and positive change.

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or an equivalent field is required.
  • Possession of an active and unrestricted licensure as a Professional Counselor (LPC) in the state of Colorado.
