Calhoun Families First Center is actively seeking a Full-Time Licensed Clinical Social Worker (LCSW) to join our dedicated team, focused on delivering compassionate care to our patients in McDonough, Georgia. Work Environment

This remote, full-time position allows you to maintain a flexible work-life balance while engaging in meaningful work directly supporting patients in our hospice center. Key Responsibilities
  • Conduct comprehensive psychosocial assessments tailored to each patient, determining their emotional, social, and financial needs.
  • Collaborate with healthcare professionals to create integrated care plans, focusing on the holistic needs of each patient.
  • Provide individual and group counseling sessions that address challenges related to grief, adjustment, and coping mechanisms.
  • Facilitate coordination with community organizations to develop effective post-discharge care plans, ensuring continuity and support after treatment.
  • Offer education and emotional support to patients and families regarding available treatment options, resources, and community support.
  • Provide crisis intervention services as needed, ensuring rapid response to the emotional and psychological needs of patients and families.
  • Maintain accurate documentation of patient interactions and care plans in accordance with hospital protocols and legal standards.
  • Engage in hospital committees, contributing your expertise on social work matters to enhance our service delivery and patient experiences.
Patient Population / Client Focus

The primary population you will work with includes patients within the hospice setting, many of whom are dealing with terminal illnesses. You will offer crucial psychosocial support to both patients and their families, helping them navigate emotional challenges and prepare for end-of-life decisions with dignity and peace.

Qualifications
  • A Master’s Degree in Social Work from an accredited program.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW).
Preferred Experience
  • At least two years of professional experience in a healthcare or hospital setting, with a focus on patient-centered care.
  • Strong knowledge of medical terminology and mental health disorders, along with skills in crisis intervention techniques.

Compensation and Benefits

We offer a competitive salary range of USD 70,000 to 100,000 annually, relative to your experience and qualifications.
