Job description

As a Registered Nurse, you will provide compassionate care at the bedside of our patients every day. You will develop and maintain positive relationships with our patients, guests, other team members, and medical staff by striving for excellence in service and quality. You will be responsible for:
* Conducting virtual assessments, triage, and follow-up visits via video, phone, or secure messaging.
* Monitoring patient status using hospital telehealth systems and escalate concerns to bedside staff & providers.
* Conducting virtual patient assessments, focusing on pain management, mobility, and discharge readiness.
* Providing evidence-based nursing interventions virtually / remotely.
*Assisting with medication reconciliation, admission & discharge planning, and chronic disease management.
* Documenting all patient interactions in the electronic health record (EHR) in compliance with hospital standards.
* Providing real-time consultation to staff for clinical decision-making.
* Supporting rapid response teams virtually by reviewing patient data and escalating concerns.
* Partnering with physicians, bedside nurses, and allied health professionals to coordinate patient care.
* Supports transitions of care between inpatient, outpatient, and community settings.
* Educating patients and families on treatment plans, medication adherence, and lifestyle modifications.
* Advocating for patients need to ensure equitable access to virtual care services.
What you will need:
* ASN Required, Bachelor of Science in Nursing (preferred)
* Current RN License to Practice in the State of Georgia
* Minimum 3 years of RN experience in an acute care hospital. Critical care cardiac experience (preferred) CAVRN within one year of hire or 1000 hours VC
