Are you an experienced Registered Nurse looking to make a meaningful impact through care coordination and community-based nursing? We’re partnering with a mission-driven healthcare organization to offer a Hybrid RN Case Manager opportunity serving members throughout Brooklyn. This role combines field-based nursing with remote care management, giving you the opportunity to support older adults and individuals with disabilities while promoting safe, independent living.
Position Overview:
The RN Case Manager is responsible for providing comprehensive care coordination, including in-home assessments, care planning, advocacy, service authorization, and ongoing monitoring to ensure members receive the appropriate services based on their individual health and functional needs.
Responsibilities Include:
  • Conduct in-home assessments and monitor members' health, functional, cognitive, and environmental needs.
  • Develop and implement individualized, patient-centered service plans.
  • Coordinate care across hospitals, nursing facilities, home care, and other healthcare settings.
  • Authorize covered services and ensure services are appropriate and cost-effective.
  • Collaborate with members, families, caregivers, physicians, and interdisciplinary teams.
  • Advocate for members and help them maintain the most independent living situation possible.
  • Complete required care management documentation, assessments, and re certifications.
  • Coordinate safe and appropriate discharge plans.
  • Identify members with complex care needs and participate in intensive care management discussions.
  • Respond to member requests, grievances, and care management needs within required time frames.
  • Connect members with appropriate community and state resources.
     
Qualifications:
  • Current, unrestricted New York State RN license required.
  • Associate's Degree in Nursing from an accredited nursing program required.
  • Bachelor's Degree in Nursing preferred.
  • Minimum 2 years of RN experience required.
  • Clinical experience in geriatrics and/or Managed Long-Term Care (MLTC) preferred.
  • Strong assessment, critical thinking, decision-making, and problem-solving skills.
  • Strong organizational and time-management abilities.
  • Ability to work independently and collaboratively with a multidisciplinary team.
  • Bilingual speaking and writing skills preferred.

Important Details:
Field Schedule: Approximately 6 home visits completed in 1 day
Remote Work: Care coordination, documentation, member follow-ups, and care planning
Training Location: Brooklyn Office
Travel: Over 50% mobile; regular travel throughout the assigned territory
Driver Requirement: Valid driver's license, personal vehicle, and verifiable auto insurance required
Salary: $100,000–$135,000 per year, based on education, training and experience
