Job description

The Moments Hospice Triage RN is a key member of the interdisciplinary team, providing after-hours clinical support to patients and families. In this fully remote role, you will assess needs, provide guidance, and coordinate care using the nursing process and Telecare protocols.

This position is ideal for an experienced hospice RN who is confident working independently while delivering compassionate, patient-centered care.

What You’ll Do

  • Provide after-hours triage, clinical assessment, and interventions via phone
  • Support patients and families with symptom management and end-of-life care needs
  • Coordinate care by collaborating with physicians, facilities, and internal teams
  • Dispatch visits and coordinate services with vendors as needed
  • Deliver clear, compassionate education and support to families
  • Accurately document all interactions, care plans, and outcomes in the EMR
  • Review team documentation for accuracy and compliance
  • Partner with Telecare staff to ensure positive patient outcomes
