Overview

We are seeking a highly skilled RN or LPN to serve as the remote support hub for our in-house Nurse Practitioners (NPs) practicing in Skilled Nursing Facilities (SNFs) as a Clinical Support Nurse.

Your mission is to fundamentally improve NP efficiency and patient outcomes by owning a powerful combination of clinical, data, and communication tasks. You won't just be handling logistics; you'll be performing clinical triage, educating families, managing critical program data, and coordinating complex care. By serving as a trusted clinical extension of our NPs, you will allow them to focus their time on top-of-license medical decision-making.ย

Responsibilities

As the central point of contact, you will:

  • Serve as the Clinical Communication Hub:
    • Act as the first line of response for our NPs, skillfully managing and triaging facility communications (e.g., medication refusals, lab delays, pharmacy questions).
    • Conduct structured telephonic or message-based triage for acute, non-emergency symptoms, escalating to the NP with clear clinical recommendations.
    • Act as a key contact for families, providing care plan education and clarification on the NP's behalf to ensure understanding and alignment.
  • Own the End-to-End Specialist Referral Process:
    • Manage the full specialist referral lifecycle, from initiating specialist outreach to tracking referral status and ensuring a closed loop.
    • Compile and send all requested clinical documentation to specialist offices.
    • Coordinate patient transportation with facility staff, ensuring no logistical details are missed.
  • Drive Clinical Data & Quality Insights:
    • Proactively locate and synthesize critical patient information from Health Information Exchanges (HIE) as requested by NPs.
    • Support program-specific tracking (e.g., Chronic Care Management (CCM) enrollment, consents, and time).
    • Perform data abstraction from the EHR to support quality dashboards and reports that are shared with leadership.

Qualifications

Must-Haves:

  • Active, unencumbered RN or LPN license in Maryland
  • Minimum of 2 years of direct nursing experience in a Skilled Nursing Facility (SNF) or Assisted Living Facility (ALF)
  • Exceptional organizational skills and meticulous attention to detail
  • Strong, professional communication skills (both written and verbal)
  • A self-starting mindset with the proven ability to work independently, manage priorities, and solve problems in a remote setting

Preferred:

  • Active licensure in Maryland (MD)ย 
  • High proficiency with Electronic Health Records (EHR) and HIE systems
