Bliss Reformation And Wellness Center, located in Hialeah, Florida, is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team in a full-time, remote capacity. This role plays a pivotal part in enhancing the mental health and emotional well-being of our clients, particularly focusing on the needs of seniors. This position offers the flexibility of a remote work setting, with full-time hours Monday through Friday from 8:00 AM to 4:30 PM. Flexibility in scheduling is paramount, as occasional evening or weekend hours may be required based on client needs. • Conduct comprehensive assessments of seniors' psychosocial needs, integrating emotional, mental, and social factors into your evaluations.
• Collaborate closely with clients and their families to craft personalized care plans that reflect their specific goals and preferences for improved well-being.
• Deliver compassionate counseling and emotional support to seniors and families as they navigate transitions and challenges in their lives.
• Act as an advocate for clients’ rights and help them access necessary community resources to ensure they receive appropriate support and services.
• Coordinate with interdisciplinary teams that include nurses, caregivers, and other healthcare professionals to enhance the overall care and service delivery.
• Conduct regular client visits to monitor well-being, evaluate living conditions, and identify any safety concerns that need to be addressed.
• Facilitate effective communication and resolution strategies among family members involved in the senior client’s care.
• Educate clients and their families on available community resources and support groups that can enhance their social connections and overall quality of life.
• Maintain detailed documentation of client assessments, care plans, progress notes, and relevant interactions, ensuring accuracy and compliance with ethical standards.
• Stay abreast in relevant social work practices, policies, and regulations to ensure that our services are up to date and ethically sound.

• A Master's degree in Social Work from an accredited institution is required.
• Possession of a current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• At least two years of experience in a relevant social work role, preferably within a community health or geriatric care setting.
• Demonstrated knowledge of social work principles, particularly those relevant to aging and available community resources.
• Exceptional assessment, counseling, and problem-solving skills necessary for effective client interactions.
• Strong ability to empathize, demonstrate compassion, and establish rapport with seniors and their families.
• Excellent communication and interpersonal skills, enabling teamwork and collaboration with various interdisciplinary teams.
• Proficiency in electronic health record (EHR) systems and standard computer applications.

• Experience working specifically with the senior population and understanding their unique mental health challenges.
