This position allows for remote work within a flexible schedule, focusing on comprehensive evaluations and direct client interactions. The work hours typically run Monday through Friday from 8:00 AM to 4:30 PM, with the potential for occasional evening and weekend hours based on client needs. The role requires adaptability for on-call duties to respond to urgent client situations and offers a supportive, collaborative environment through virtual teamwork.

• Conduct meticulous assessments of seniors' psychosocial needs, factoring in emotional, mental, and social dimensions.
• Collaborate closely with seniors and their families to create tailored care plans that reflect their goals and preferences.
• Provide counseling and emotional support to clients and their families, assisting them through significant life transitions such as health changes or loss.
• Advocate effectively for clients, ensuring they gain access to necessary community resources and support services.
• Work with a multidisciplinary team, which may include nurses, caregivers, and other healthcare professionals, to coordinate comprehensive care.
• Regularly monitor clients' well-being through virtual check-ins or remote assessments, addressing safety concerns and improving overall living conditions.
• Facilitate open communication to resolve conflicts among family members involved in the care, aiming for a cohesive support network around the senior.
• Enhance client knowledge of community resources and services, providing education on available support groups and programs.
• Maintain accurate and compliant documentation of client assessments, progress notes, and care plans within our electronic health record (EHR) systems.

The LCSW will primarily work with senior clients who may be experiencing a variety of challenges, including substance use disorders, mental health issues, or general health declines. The detox center's mission is to support clients through their recovery processes while emphasizing dignity, respect, and person-centered care.

• A Master’s degree in Social Work from a program accredited by the Council on Social Work Education.
• A current and valid state-level licensure as a Licensed Clinical Social Worker (LCSW) in Kansas.
• A minimum of two years of experience in social work, ideally within settings serving geriatric populations or community support roles.
• Extensive knowledge of aging-related issues, social work principles, and local community resources.
• Strong assessment, counseling, and problem-solving skills tailored to senior client needs.
• Ability to communicate effectively with a wide range of individuals, including clients, families, and interdisciplinary teams.
• Competency in utilizing EHR systems and technology for case management.

• Experience in a detox or rehabilitation setting, with insights into addiction recovery processes.
• Strong interpersonal skills to build trust and rapport with seniors and their families.
• Previous involvement in developing community outreach programs or resources aimed at supporting elderly populations.
