Job description

Position Summary

The Triage Licensed Practical Nurse (LPN) is responsible for providing timely and compassionate telephone triage support to patients under the direction of a licensed provider. This position serves as a clinical resource by gathering patient information, assessing symptoms within the LPN scope of practice, determining appropriate next steps using established protocols, coordinating with providers, and ensuring accurate documentation within the electronic medical record (EMR).

In addition to telephone triage, the LPN will assist with Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Emergency Room (ER) and hospital discharge follow-up to promote continuity of care and improve patient outcomes. This is primarily a desk-based position requiring excellent communication skills, sound clinical judgment, strong organizational abilities, and the capacity to manage multiple priorities in a fast-paced outpatient setting.


Essential Duties and Responsibilities
  • Monitor triage phone queues throughout the day.
  • Assess patient concerns within the LPN scope of practice using approved triage protocols and escalate to the provider when appropriate.
  • Process prescription refill requests according to provider-approved protocols.
  • Respond to patient portal messages as assigned.
  • Complete Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) patient outreach and documentation.
  • Review daily Emergency Room and hospital discharge reports.
  • Perform discharge follow-up calls and coordinate post-discharge appointments.
  • Communicate provider recommendations and care instructions to patients.
  • Coordinate same-day appointments when clinically appropriate.
  • Follow up on outstanding patient concerns until resolution.
  • Accurately document all patient interactions in the electronic medical record (EMR).
  • Maintain patient confidentiality and comply with HIPAA and organizational policies.
  • Collaborate with providers and clinical staff to ensure continuity of patient care.
  • Perform other duties as assigned.

Required Qualifications
  • Current unrestricted Licensed Practical Nurse (LPN) license.
  • Graduate of an accredited practical nursing program.
  • Minimum of two (2) years of clinical nursing experience preferred.
  • Previous experience in primary care, family medicine, internal medicine, emergency department, urgent care, ambulatory care, or telephone triage preferred.

  • Ability to multitask, prioritize, and work independently.
  • Proficient with electronic medical record (EMR) systems.
Preferred Qualifications
  • Telephone triage experience.
  • Ambulatory care or primary care nursing experience.
  • Experience with Chronic Care Management (CCM), Remote Patient Monitoring (RPM), or Transitional Care Management (TCM).
  • Knowledge of evidence-based triage protocols and patient education.
  • Bilingual (English/Spanish) skills are a plus.
