In this position, you will work remotely, connecting with patients and their families in a comfortable, compassionate manner that ensures trust and effective communication. While you will primarily engage in virtual consultations, you will collaborate closely with an interdisciplinary team of healthcare providers, including physicians, nurses, and chaplains, who are dedicated to holistic patient care. • Conduct detailed psychosocial assessments to identify the emotional, financial, and social needs of patients, guiding appropriate interventions.
• Provide both individual and group counseling services, focusing on issues such as grief, adjustment to illness, and coping strategies.
• Collaborate with medical and nursing teams to design and implement comprehensive care plans that address the psychosocial aspects of patient care.
• Coordinate with community organizations to facilitate access to post-discharge services, ensuring smooth transitions and continuity of care.
• Offer educational resources and emotional support to patients and their families concerning treatment options and available resources.
• Engage actively in interdisciplinary team meetings and hospital committees, contributing your expertise on social work and mental health matters.
• Implement crisis intervention strategies as needed while maintaining detailed and accurate documentation in alignment with hospital policies.
• Develop discharge plans that connect patients with essential community resources, enhancing their support systems during transitions.

This position focuses primarily on patients receiving hospice care, who often experience complex emotional and psychosocial challenges. You will work closely with terminally ill patients and their families, providing essential support as they navigate end-of-life issues. Understanding the unique needs of this patient population is crucial, as it requires sensitivity, compassion, and a thorough understanding of medical terminology and mental health issues.

• A Master’s degree in Social Work from an accredited institution.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Virginia.
• Knowledge of diverse mental health disorders and crisis intervention techniques, paired with familiarity with hospice care practices.
• Exceptional communication, interpersonal, and collaboration skills that facilitate effective teamwork and patient engagement.

While not mandatory, candidates with two or more years of experience in a healthcare or hospital setting will be highly regarded. Experience with hospice care or working with terminally ill patients is advantageous and will enhance the depth of support provided to our patient population.

Great Lakes Consultants Services offers a competitive salary range of USD 70,000 to 100,000 per year, commensurate with experience and expertise. Additional financial incentives including a sign-on bonus may be available. Our comprehensive benefits package includes:
