Registered Nurse (RN)

Flexible Schedule Halcyon Home Care | North Fulton & South Forsyth (Alpharetta, Roswell, Johns Creek, Milton, Cumming, and nearby communities)

Pay: $75–$100 per visit, based on visit type and case

Halcyon Home Care is a locally owned, RN-led home care agency based in Alpharetta. We are looking for a compassionate, reliable Registered Nurse to join our growing team. This is a flexible, per-visit role, ideal for an RN who wants meaningful one-on-one care in the home and a schedule that works around their life.

Position Overview The RN conducts client assessments, develops and oversees individualized care plans, and provides periodic supervisory visits and skilled nursing visits as needed, while supporting caregivers to ensure safe, high-quality care in the home. Our office handles scheduling, logistics, and documentation support so you can focus on the nursing.

Key Responsibilities

  • Conduct initial and ongoing RN assessments in the client's home
  • Develop and update individualized care plans
  • Provide periodic supervisory visits and skilled nursing visits as needed
  • Monitor for changes in condition and coordinate with physicians and families
  • Provide clinical guidance to caregivers (and LPNs, if applicable)
  • Ensure accurate documentation and compliance with care standards
  • Participate in periodic reassessments and ongoing care planning

Qualifications

  • Active Georgia RN license in good standing (a compact license is accepted only if Georgia is a permitted state; traveling-nurse licenses are not accepted)
  • 1–2 years of clinical experience (home care or community nursing preferred)
  • Experience with patient assessments, care planning, and skilled nursing procedures
  • Strong clinical judgment and the ability to work independently in a home setting
  • Excellent communication and documentation skills
  • Reliable transportation for home visits in the North Fulton / Forsyth area
